TRAVEL WAIVER FORM
PLEASE NOTE:  When form is completed and approved, your student athlete will not be expected on the bus.  This is not to be filled out as a "Just in case they don't want to ride the bus".  If the form is filled out, they will not ride the bus home.  Also, if an issue arises the adult listed will be contacted.  Therefore, we can not have an excess of adults listed.
